Community Mental Health Practitioner - Lancashire & South Cumbria NHS Foundation Trust

You should have a collaborative and enthusiastic approach as well as passion and commitment in developing and promoting recovery based approaches for adults with severe and enduring mental health needs.&lt;br&gt;&lt;br&gt;The post requires that you are a good communicator, and are able to adopt a flexible, adaptable and proactive approach to your work.&lt;br&gt;&lt;br&gt;The main job role is to perform duties as a Community Mental Health Nurse and to undertake assessments for people who are referred into the service for physical health monitoring when commenced on anti-psychotic medication including depot and Clozapine.&lt;br&gt;&lt;br&gt;As the Band 5 Community Mental Health Nurse you will manage a caseload of service users with a range of complex needs. You will develop care plans and deliver evidence based interventions and treatments to patients alongside managing risks in the community for patients who are under the CMHT for shared care and depot and Clozapine monitoring. You will be working under the supervision of the clinical lead and have support from the Community Mental Health team multi disciplinary team. You will also have the opportunity to supervise junior staff.&lt;br&gt;&lt;br&gt;This role involves managing unplanned patient work within the team, responding to escalation of risk/crisis situations and if required to step up care to a CMHT Band 6 Practitioner to assess.&lt;br&gt;&lt;br&gt;You will benefit from encouragement to continue to develop your own specialist knowledge and competence.
