  • Notify the pharmacist/provider of a missing prescription
  • Notify the pharmacist/provider of a request for a medication refill
  • Call pharmacies to confirm prescriptions were received and picked up
  • Call patients to remind them to pick up their medication
  • Request records from clinics and facilities when necessary
  • Send referrals to other disciplines to coordinate care when necessary
  • Enroll patients into criteria-based pharmacy programs and document in the electronic medical record using established formats in a timely and accurate manner
