  • The post holder will care for patients in the community and work within the Ambulatory Outreach Team (AOT).
  • Assist in the development, implementation, delivery and evaluation of the AOT [Acute Hospital at Home (AHaH) & Complex Outpatient Parenteral Antimicrobial Therapy (COPAT)] function and contribute in further development and streamlining of the AOT.
  • As a senior nurse, the post holder is expected to work and provide specialist input, and flexibly rotate within both services.
  • The post holder is expected to work autonomously and support other team members.
  • Care for patients in their home environment, support and improve the patient discharge process for patients who can safely be managed at home with a defined care plan.
  • The post holder needs to be able to perform holistic clinical assessment for Admission Avoidance patients (training will be provided) at the patient’s home.
