Responsibilities

  • Support service users in our newly commissioned Enhanced RECONNECT Service.
  • Work within a psychological framework to aid the recovery and risk management of service users returning to community settings from prisons or secure hospitals.
  • Support the delivery of evidence-based recovery-focussed interventions and support individuals to achieve positive outcomes and recovery goals based on the multidisciplinary team assessment and formulation of the individual’s needs.
  • Engage the service users via in-reach into the prison/hospital before they are released/discharged into the community and continue to work with them in the community to promote recovery.
  • Work proactively with other professionals and services, such as GPs, substance use agencies, DWP, Housing providers, families & carers, friends and peer mentors to support recovery plans.
