Requirements

  • - Have a good understanding of the normal parameters of physical and mental health, to be responsive and proactive to deteriorating health
  • - Provide effective professional support to the physical health team, including sharing best practice and role modeling to all staff
  • - Ensure appropriate physical health observations and blood tests are taken and actioned for all patients who are eligible for an annual physical health check

Responsibilities

  • - Provide high quality clinical support to junior colleagues and to act as a clinical expert. This means working in accordance with evidenced based care and interventions in conjunction with locally agreed locally agreed protocols and policies.
  • - The post holder will contribute to assessment of both physical and mental health needs. With a focus on improving the physical health of people of severe mental illness, the post holder will understand the relationship between physical and mental wellbeing.
  • - Have a good understanding of the normal parameters of physical and mental health, to be responsive and proactive to deteriorating health.
  • - Provide effective professional support to the physical health team, including sharing best practice and role modeling to all staff.
  • - Ensure appropriate physical health observations and blood tests are taken and actioned for all patients who are eligible for an annual physical health check. This will involve liaison with GPs, practice nurses, practice managers and other primary care staff.
  • - Assess patients holistically, and to support junior members of staff with the assessment of lifestyles, exercise, activities, mental health, nutrition, smoking, alcohol, drug use, and sexual health. Giving advice on healthy lifestyles following evidenced based practice and signposting our service users to other services.
  • - Facilitate referral to external services as required including social services and secondary mental health services.
