Responsibilities

  • Reporting to the Manager or designate, the Interdisciplinary Clinical Leader provides leadership innovation and support to the clinical program area, to provide interdisciplinary collaborative person-centered care to patients.
  • The Interdisciplinary Clinical Leader works closely with the Program Leaders, Physician Leaders, Professional Practice Coordinators, Physicians, Nurses, Social Workers, Dietitians, and other team members to promote person-centered care to the patients of the One Door Chronic Disease Centre.
  • Key duties include developing and fostering healthy interdisciplinary team processes that develop a person-centered continuum of care, program planning and evaluation – ensuring practice is interdisciplinary and evidence-based, participating in the recruitment, selection, and orientation of new team members, supporting a quality approach to care delivery, coordinating and providing clinical care for patients.
