Responsibilities

  • The role involves working across Liverpool community teams (North, Central, South, Neuro, Rehab at Home, Care homes and Falls) on a rotational basis to provide support to our patients in their homes and other community settings, which will enable you to develop skills and flourish within your career.
  • The post holder will be expected to effectively manage a caseload of patient’s co-morbidities as an autonomous practitioner, using evidence based patient centred principles to assess, plan, implement and evaluate interventions of the patient group.
