  • The primary function of this role will be to work with the Occupational Therapy, Physiotherapy & District Nursing Services within Intermediate Care to deliver therapeutic treatment programmes to service users in the ward setting or their own homes, preventing hospital admission and supporting early discharge from hospital.
  • You will have an SVQ Level III in Health and Social Care or equivalent.
  • Experience working as a support worker would be desirable.
  • It would be desirable to have experience working with older people as they are the main group of service users referred.
  • Working as part of a supportive multidisciplinary team you will be expected to carry out ongoing treatment programmes for a mixed caseload of mainly older people.
  • You will have good organisational skills, excellent communication skills and be able to work independently as well as within the team.
  • There may be occasions where you will be required to work in one of the other community hospital sites.
  • You will require to work 15 hours flexibly over 7 days.
