Responsibilities

  • - Work alongside ACP, Physio, and Nursing staff to provide direct specialist clinical care to people with severe, complex, and debilitating chronic respiratory disease in the home setting.
  • - Provide early access to Pulmonary Rehabilitation following an acute exacerbation of chronic lung disease.
  • - Improve condition management, health outcomes, and reduce further urgent emergency care usage.
