  • You will work with the Multi-Disciplinary Team: a spectrum of clinical professionals which includes Nurses, Doctors, Occupational Therapist and support staff providing Intensive Support to patients living with dementia and older adults with complex mental health needs, whom without intensive support would be at risk of needing a hospital admission.

Responsibilities

  • To work collaboratively with key partner organisations, Service Users, carers, clinicians and other practitioners within the Multi-Disciplinary Team to deliver high quality, recovery focused care interventions.
  • You will be responsible for assisting in the management and care of Service Users based upon individual needs, under the care planning and guidance of senior qualified staff.
  • To support the service and Associate Practitioner by conducting physical health assessments for service users.
  • Provision of practical support to Service Users in performing day to day activities, guidance and education in accordance with the agreed care plan and for a dedicate period of time.
  • Produce accurate and timely documentation, conduct and manage the maintenance of records on all Service User under your care in line with Trust policy.
