Responsibilities

  • Address the needs of individuals to bring about improvement in the health and wellbeing of patients through the delivery of evidence-based care
  • Manages the ward on a regular basis, recognising and responding effectively to the changing nature of the workload.
  • Model excellent practice through effective assessment, planning, implementation and evaluation of care.

  • Initiate and develop plans for health promotion with individuals involving parents/carers and multidisciplinary teams.

  • Negotiate sensitively with patients, relatives and carers regarding care and discharge planning.

  • Liaises with ward manager to manage sickness and staffing.
  • Liaise with site managers, bed managers and matrons to and multidisciplinary team regarding admissions, discharges and flow of patients between departments
  • Demonstrate use of IT skills to complete details of admission/discharge of patients using a computerised record system
  • Write precise, accurate and timely nursing records, reports and statements.
  • Listen actively and respond appropriately to information given.
  • Leads shift handovers ensuring relevant detail is communicated to the team

  • Works towards achieving the trust objectives as laid out in the Nursing and Midwifery Strategy. Facilitate and support the clinical educator and support the development of other staff to undertake a link nurse role as designated by the Ward Manager.
  • Lead and delegate mandatory and other audit as required by the Trust
  • In conjunction with other clinical staff, sets, monitors and evaluates standards of care for patients in all areas.
