Responsibilities

  • Provide leadership and support for the Haematology Cancer CNS Teams cross-site
  • Work autonomously and demonstrate advanced practice by managing a caseload of patients and demonstrate advanced practice in the holistic assessment
  • Lead the development of Personalised Care for haematology cancer patients which will include the further development of the Supported Self Management programme and play an active role in the delivery and design of ‘Moving On’ and Health & Wellbeing Events
  • Promote collaborative working relationships and effective communication between all members of the MDT to improve the management and outcomes of patients within the specialist area
  • Facilitate effective communication and co-ordinate referral of patients and carers to appropriate support services and community Trusts to promote a seamless service and thus reduce/avoid admissions and length of stay to the acute trust
  • Lead and manage the Haematology Cancer CNS Nursing Teams cross-site on a daily basis, ensuring that the teams provide an excellent service to the Trust through evidence based practice, education, policy and guideline development
  • Continually develop, implement and evaluate the Haematology Cancer Service to improve and maintain standards of patient care and promote a seamless service within a multi-professional framework
  • Lead developments in practice consistent with the patient care priorities set nationally and locally
  • Development of new roles or service redesign will take place in line with best practice and evidence based care
  • Develop and manage nurse led clinics to streamline the haematology cancer services to improve patient experience and develop effective pathways
  • Maintain a high profile in clinical practice, promoting clinical excellence, initiating learning opportunities and promoting effective learning environments in all ward areas and clinic areas
  • Accountable for ensuring the highest standard of clinical care is provided within the relevant setting
