Responsibilities

  • To work as part of a specialist children and young people’s mental health team to provide intensive Tier 4 support and interventions for young people and their families.
  • As a Mental Health Practitioner you will be responsible for undertaking assessments for young people referred for Tier 4 CAMHS intensive interventions.
  • As well as face to face assessments the role involves all the gatekeeping activities as well as offering consultations to our Tier 3 partners.
  • In addition to gatekeeping activities the post holder will undertake planning and facilitating discharge from Tier 4 services.
  • To work as part of a team on a rota basis including unsocial hours.
  • To promote and uphold CWP Trust Values.
  • Be responsible for undertaking the assessment of young people referred to Tier 4.
