Requirements

  • - Follow up older adult patients in the community who has been assessed by the team and needing support for their social care, physical care or mental health care needs.
  • - See to patient physical health needs that is impacting on patient's mental health i.e blood pressure, weight loss, stress
  • - Support carers and advising them about available support in the community
  • - Carry out simple care needs assessment and record keeping on epjs
  • - Work autonomously with support from their supervisor/ manager of the team
  • - Risk assess patient in the community for the older adult population
  • - Support community patient to access their GP in order to reduce inequality in services
  • - Support carer and signpost them to community support available
