  • We are looking for Clinical Triage Clinicians to join the Dudley Clinical Hub (Single Point of Access) offering telephone triage, clinical advice and signposting.
  • The successful applicant will join an integrated, collaboratively focused team, employing a proactive clinical practice and knowledge to provide phone triage / clinical assessment skills to support urgent care for patients across our health economy.
  • The service is the single point of access for patients registered with a Dudley GP for all admission avoidance referrals from professionals and for patients requiring Health and Social Care Services.
  • The successful applicant will be expected to work flexibly throughout this period.

Responsibilities

  • To provide clinical support, assistance, and professional expertise to all healthcare professionals in support of avoiding inappropriate hospital attendances and admissions, then referring onto the most appropriate service for the patient’s needs.
  • Working closely with and acting as the referral point for the Urgent Community Response Team.
  • The primary objectives are to prevent unnecessary admission to acute care and to facilitate collaborative MDT management for patients closer to home.
  • The role involves taking telephone referrals from GP’s, West Midlands Ambulance Service, Community Nurses and all other Health Professionals.
  • The triage clinician will be expected to visit patients in their own home supported by the Medical Lead and Senior Practitioners if required.
  • This will enable the clinician to receive further training to ensure clinical skills and competencies are maintained.
