Responsibilities

  • - Lead specialist for community nutrition support team
  • - To lead on community nutrition support service development
  • - To lead on relevant medicines optimisation activities/projects around appropriate prescribing of nutrition-related products in primary care
  • - To provide assessment, treatment and management of housebound patients requiring nutrition support who may have both complex and chronic social and medical presentations affecting their nutritional intake.
  • - To provide specialist advice, support training and practical resources for the management of malnutrition, including nutritional screening, food fortification and the appropriate use of prescribed nutritional supplements for care homes and primary care professionals e.g. GPs, District Nurses, Community Pharmacists and the Integrated Care Teams
