Responsibilities

  • - The post holder will contribute to the provision of specialist palliative and end of life rehabilitation to the people of Cheshire with malignant or non-malignant conditions who have a limited life expectancy, working autonomously and as part of a team.
  • - They will deliver therapeutic interventions in their home and/or when they attend the Hospice as an out-patient, in-patient or attend day therapy, providing advice, education and support to patients, their carers and other health care professionals.
  • - They will also promote advanced care planning, management of complex end of life symptoms and patient and carer expectations whilst meeting the integrated care needs of the patient.
  • - Over recent years there has been a lot development within the service and the successful candidate will need to share the teams drive to take the service forward.
  • - You also need to be passionate about delivering quality care to people receiving palliative or end of life care.
