  • The post holder will be expected to work the within the to proved clinical remote monitoring care to patients with variety of clinical needs and work with other clinical specialist across Dorset to develop the service.

  • You will need a sound understanding of the current primary care agenda, including the management of long term conditions also with excellent communication skills and to be able to demonstrate effective leadership, decision making skills and resilience.
  • You must be able to demonstrate a high level of clinical knowledge, team working, good interpersonal skills and time management.
  • You should have experience in caseload management and be able to priorities work.
  • Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.
  • Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

Responsibilities

  • Attend integrated MDT meetings as part of the clinical team caring for remote monitored patients.
  • Work within the provided clinical remote monitoring care to patients with a variety of clinical needs.
  • Work with other clinical specialists across Dorset to develop the service.
  • Provide high quality clinical care, management of staff and case management for patients in the community setting.
  • Demonstrate a high level of clinical knowledge, team working, good interpersonal skills and time management.
  • Have experience in caseload management and be able to prioritize work.
