Responsibilities

  • The post holder will support young adults who may be struggling with a range of difficulties related to living with diabetes, as they make the transition to adult services.
  • The post holder will have scope to develop the service, with the support of more senior colleagues and in collaboration with service users.
  • The post will have a base at Mile End Hospital, but the job will involve working across all of the Barts Health sites (Whipps Cross Hospital, Newham University Hospital, the Royal London Hospital, and St Barts Hospital), and ensuring that standardised, high quality care is delivered across the Trust.
  • The post holder will be part of a core Young Adults diabetes team that will diabetes transition and young adults services across Barts Health.
  • The post holder's primary role will be supporting high risk young adults and working with the teams social worker, youth worker and psychologist to safeguard these patients and engage them.
  • The post holder will develop specialist skills enabling them to provide management plans, treatment advice, and support for young adults (age 16-25) with diabetes and their carers.
