Responsibilities

  • The job role is to assist the Care Co-ordinators within the Intermediate Care Service who will supervise and support the post holder to ensure referrals are managed in a timely and safe manner
  • The job is focused on receiving new referrals into the relevant Intermediate Care Service, and assisting to co-ordinate and organise packages of care delivered by the local Intermediate Care Service to support patients living in the community.
  • To work closely with the relevant Intermediate Care Service’s Team Lead and senior therapy staff to promote an multi-disciplinary approach to management of patient care.
  • To work collaboratively with colleagues across the social care, health and the third sector to provide patient centred, integrated care.
