What is the role of an Integrated Care Worker (Rehab) in the Home First Intermediate Care Service?
The Integrated Care Worker (Rehab) supports patients in achieving their optimum independence by assisting with the delivery of high standards of health care, following prescribed packages of care and therapy to facilitate earlier discharge from hospital and avoid hospital admissions.
What qualifications do I need to apply for this role?
Qualifications are considered for this position, and the role may be offered as a Band 2 Development post depending on your qualifications and experience. You will be supported to develop into a Band 3.
What are the working hours for this position?
The operating hours are from 7am to 10pm, 7 days a week.
Is a driving license required for this role?
Yes, you must hold a full UK driving license and have access to a vehicle, as travel is required around the county to meet service needs.
How many patients will I be managing as an Integrated Care Worker?
You will manage your own caseload of patients who require ongoing management and support with complex needs.
Will I receive training to perform my duties?
Yes, you will receive training to use skills from various professional areas such as Nursing, Allied Health Professionals, and Medicines Management to undertake delegated tasks.
