Responsibilities

  • - Deliver care to patients closer to home and preventing unnecessary hospital admission in West Hertfordshire
  • - Work primarily under the Discharge to Assess and Rapid Response pathways
  • - Provide holistic assessment within 2 hours of referral and treatments over a 5 day pathway
  • - Support the discharge of patients from acute and community hospitals to assess and manage rehabilitation/therapy needs
  • - Facilitate a safe discharge from hospital when the patient is medically well enough to go home
  • - Respond to referrals for patients identified as requiring support for low complexity health and functional needs
  • - Ensure holistic support is provided in collaboration with other community providers in west Hertfordshire
  • - Deliver pathways across all 4 Herts Valleys localities and provide a clinical in-reach function at acute hospitals to prevent admissions and support timely discharges
