The main focus of the Ward Tracker role is to facilitate effective and timely discharge of patients no longer requiring acute care.
Who will the Ward Tracker communicate with to ensure patient discharge?
The Ward Tracker will communicate with the Hospital Discharge Team, the Discharge to Assess Hub, and the wards to ensure safe and timely discharge of all patients.
What are some key responsibilities of the Ward Tracker?
Key responsibilities include supporting the Discharge Team in facilitating timely discharges for complex patients, attending whiteboard rounds and ward rounds, managing discharge plans, assisting in discharge checklists, and transferring patients to the discharge lounge if required.
